Hi Joan,
   I worked in the Pursuing Perfection Project in Bellingham,  
Washington from 2002-2005 - helping to redesign health care to  
increase effectiveness, safety, efficiency, equality, etc.  There are  
various guidelines and standards already developed through the  
Institute for Healthcare Improvement (ihi.org), and also the American  
Academy of Family Physicians (aafp.org)  regarding patient safety  
that you might find useful.

   The challenging thing I experienced, was helping the practices  
make the changes and hold the gains, and this is where an ai approach  
can be useful, in building a culture where it is safe to admit  
mistakes (and learn from them), where people aren't blamed, but  
processes are examined and changed to improve safety.

Patient safety encompasses a wide range of factors:  medication  
reconciliation, standard practices, documentation, communication with  
patients, protocols, medical records, falls, reduction of errors,  
etc.  I'm happy to share any of my experience or findings that might  
apply to your specific situation.  Just let me know.
