I haven't tried this but it's an idea based on personal experience.

People work in networks. The people in the network should be contributing
heavily to the evaluation process. This should give 

A) An evaluation from a number of perspectives about the quality and
effectiveness of your work and ability to form working relationships.
B) It should show where you perform best which would be a good indicator of
future potential.

Your direct manager should review this review with you (they will probably
be one person in your network anyway). In Ai terms the process should focus
on strengths and successes rather than on weaknesses. If there is any
significant weakness these should be dealt with in a more timely fashion
outside of the employee valuation process.
