When we were at the NTL - AI Colloquy in Bethel this month - Joep Dejong shared with us the wall of wonder process, I have attached his electronic document. It does say at the bottom of the document Based on the work of Vaughn O'Halloran (ICA) from 1995, adapted and modified by Joep C. de Jong (JLS International) in 2000, 2005 and 2007 so I think it is the document that you want.
